Lisa Gherardini and Francesco del Giocondo had five children and she outlived her husband. She lived at least into her 60s, though no record of her death was located.

I don't know about the sitter's fate, but I learned in a recent Art History cource at my local community college, that the painting was Leonardo's very favorite. He was commissioned to do the work, but never actually delivered it. Instead, he carried it around with him, through all his travels for the rest of his life. He would touch it up from time-to-time, so it was a work in progress. I learned about the fine, gauzy veil from art history instructor too. She says it's too fine to be seen in photos, but you can make it out in person. Many artists of this period liked to paint the fine veil because it was a way of showing off their precision and skill.
