a joke on the day he had to go to the doctor for heart tests. But didn't tell you about the doc. visit. And you confronted him about it and he laughed and said he was at work and didn't even think about it. What does the laugh mean?

That he is frightened out of his mind by what the tests may reveal. I did the same thing with my wife hours before a flight for life to a hospital for a quad bypass.
What he needs is for you to take his hand, tell him that he is important to you, and that you don't want to have secrets. If he is worried, then you are there to carry part of the worry.

Maybe he was nervous and trying to find a way of dealing with his feelings...jokes help people get through the day I guess.

I agree with what Ole marble stated.He is nervous for some reason. I'd said he either doesn't want you to worry or he is just trying to prepare or protect himself from whatever he is worried about.
