Medicaid is publicly funded and there are all kinds of hoops to jump through to get coverage...and rules to be eligible. I'm not sure what you mean by "payer of last resort". Sometimes it is the ONLY insurance people are able to get.

because the government does not want to use taxpayer dollars to pay a bill if there is another means of paying it. private insurance always pays first.

Because it is WELFARE, paid for by the taxpayers.
If you have any other way to pay for it, WE shouldn't have to.
I don't care if you have to sell your car or your computer, I don't owe it to you to pay your bills!
