First, memorize the rhythm on the guitar, then play it a hundred times or so to get the muscle memory, then practice singing just a few words here and there while you strum, as you get a little more coordinated, sing more words, and eventually you'll get it. Mainly practice practice practice.

buy a chordbook. You can find complete ones at music stores with every single guitar chord you will ever see. Then find songs that have the chords over the words, and look up the chords in your chordbook as you come across new ones.
