What people CALL things varies greatly.
I think you're right that some struggle MUST occur for a war to be taking place, but I don't think the struggle must necessarily always occur on the crass physical level with guns and killing.
Think of the many nations and peoples who are engaged in a constant struggle with all their energy for years and years but yet do not physically fight. I'm sure you can think of many ways these conflicts surface: through subterfuge and stealth, in economic conflicts, as a social war of ideas, and so on.
Take the U.S. struggle with Cuba. Though it's seldom called a 'war', it is an obvious conflict of opposing ideologies. The U.S. spends millions sending propoganda into the country by radio, secretly (and overtly) supporting dissidents in the country, trying to block economic deals, and physically patrolling the border. I'd say that on some levels violence has been boiling just under the surface for some time.
And ultimately that's what any war is about - a struggle of ideas. Even if that idea is simply that one people should serve another people or give something up to another people. Obviously if everyone agreed, there'd be no reason to fight at all... the exchange would just happen. But instead each side clings to its ideas so strongly that they are willing to die rather than give them up. (Which, ironically, suggests that most Americans are neither at war with Cuba nor with terrorists. Just their government, perhaps)
THAT, to me, is what war is. Killing is just a symptom.

It could be 3 sides or even more. There could even be betrayal in a same side then chaning their minds etc.
