This is just a guess but Bhutan, Iran, Saudi Arabia and Vatican City. They are the first four theocracies I could think of, though I'm not sure about Bhutan.
As an atheist I wouldn't say this is a good thing.

None legally. That's a direct violation of Article VI of the Constitution:
"no religious test shall ever be required as a qualification to any office or public trust under the United States."
