As a poll worker, I can tell you for certain that your local Board of Elections can send you that application for an Absentee Ballot. You may be able to download the form from their website. I have received at least 3 applications in the mail (one from each party and on from the BOE). Some states require you to have a reason to vote absentee. Ohio just became on of the states that allow absentee voting for any or no reason.

Contact your city or town hall. Probably the board of election commissioners, but the city clerk can probably help you. They will send you an absentee ballot.

Contact your state board of elections. You may have to provide copies of your ID in order to vote, depending on the state. Unless there is a compelling reason, you may want to consider voting in person, just to be sure your vote doesn't get "misplaced."
