Islands are inhabitable (ie: there is soil, wildlife, plants, trees, etc.) whereas a big rock sticking out of the water is just that... a big rock sticking out of the water. Over many years, one of these big rocks in the water could turn inhabitable.

The Hawaiian Islands are a good example of this. They were formed from volcanic activity and evolved from volcanic rock to inhabitable islands.
