If you are talking about the medical record, yes. Doctors are required to document all patient visits, either by handwriting or dictation, dictation being the most common. Many hospitals already contribute to a database that is shared between hospitals. This information is protected by HIPAA, making all patient information confidential. When you visit a doctor, you will have to sign a HIPAA form, verifying your provider made you aware of this law.

If the doctor makes a claim to your medical insurance it gets entered into the system. Doctors also maintain records of your visit. I am not sure what do you mean by National Database If you are talking about computerized system to keep track of who is visiting which doctor I guess it isn't materialized (I not 100% sure) because your visits are still private (protected by doctor patient privileges) and it cannot be availabel in a national database where someone can access it.

In order for someone especially law enforcement to look into our records I guess they have ask our permission to look into our records or come with a court order to do so and ask the doctor to part with the data!. (Thanks to Law and Order)

Other than this I dont see any national database available at this time that keeps track of each and every doctor visit of patients!
