This will happen to me shortly, well, if it's a boy!! He is your Great Nephew. And I don't know about you but I'm not looking forward being called Great Aunt ;-(

Everyone here is right. Some use the term great-nephew and some use grand-nephew. You can use nephew if you prefer, just as some call their "great-aunt" their "aunt" and their "great-nan" their "nan".
There are no strict rules.

It is better that he calls you uncle or grandpa according to your age!
Generally most of the relationships don't carry any partricular addressing name after nephew - niece. Either one has to add grand or great with it to support that relationship that grows further!!!
