For me it is when people chew with their mouths open, clip their nails outside of the bathroom or in private, or eating while talking on the phone. ICK!!! that and scraping nails on a chalkboard!

chewing w/ their mouth open drives me nuts/chewing like a cow !! , biting their nails...even nastier when they spit it out on the floor !

Chewing with your mouth open, a clock that ticks in the silence, being inside and hearing people bounce a ball for hours, and hearing the muffled sound of a TV or stereo (bass).

The sound of a Roto-Zip saw cutting Hardi-board. Hardi-board is a cement-based siding used on houses. The sound makes my fillings vibrate and nearly loosens my bowels.

People who grind their teeth and people hacking.. I hate the sound of someone who is really sick coughing with their mouths open and are just exaggerating the sound..
