Jazz dance is very presentational and often has a higher degree of athleticism. While much of the movement vocabulary can be 'original' there are plenty of stock moves. Jazz dancing is often compared to the style seen on Broadway due to the cross referencing of musical theater, jazz music and dance.
Both jazz greats Miles Davis and John Coltrane played their versions of show tunes (like My Favorite Things). Cole Porter, the song writer wrote jazz songs (now standards) that were performed on Broadway. And, finally, Jerome Robbins and Bob Fosse were heavily influenced by jazz music and it's echoed in their choreography.

Modern Dance is very different from Jazz Dance. This style may not even have music! It often has a technique connected to it like "release and contraction", Graham, Horton, Cunningham are all techniques. MD can be presentational but is often introspective. There might be a political message in the choreography and is frequently viewed as much more experimental than Jazz.

The moves are not really the same, however, both will likely have ballet technique as the source of body training.

Yes, the music is the main distinguishing feature. Given the beat of the music, the moves that go with it are likely to have a sexier nature to them. This is why wise dance teachers will put a minimum age (like 10) on when children can start studying jazz dance.

Jazz emphasizes the bodies release into gravity, like a bungy-cord type feeling and moving the body to music. Modern dance is about innovation and freedom of movement and expression.

In Jazz you wear Jazz Shoes and in Modern you don't wear shoes at all and you dance barefoot. Besides that, there is little difference.
