Get online and search for the State where you were born. Then go to their State Government page and you should find records, vital statistics etc. Then you should be able to go from there, some states you can apply online for a new one, others you have to call.
Good Luck, I've had to do this twice, once because I lost mine and the second because my daughter was cleaning up and threw away our records envelope.

There is no replacement for lost of Birth Cert. However, you can get a re-print.
You'll need to make a police report, then submit your verification / legal documents. Including Parents' marriage certificate at the Immigration Building, Registry of Deaths and Births Department.
You'll need to pay an administration fee, $50.

Birth certificates are filed in the state capitol courthouse. You need to contact the county clerk, and there will be a fee (it varies state to state). Allow about 6-8 weeks.

Birth Certs are re-issued by the county or hospital that you were born in. You just have to answer some questions, pay $20 and wait about 2 weeks.

Yes, Just type in "birth certificate and wherever you were born" it will bring you where you need to go. You can usually order them online and pay with a credit card.

If you are a Singaporean, first make a report at the nearest Police Post. Then proceed to ICA building at Lavender Street to get another copy. If I am not wrong, replacement fee is $30.00. Please check it out.

You have to go to the County you were born in, send them proof of identification and a check and then wait and you will get a new one, It should have a raised stamp on it to prove it is authentic.
