Generally it is a two fold exam. They draw your blood and also check you manually. If you think having someones gloved finger stuck up your rectum is a time to make a snappy remark or impress a hot assistant you are sick.

The doctor doesn't need an assistant nor does he need guidance. Unless you want it, the assistant does not have to be present. I've had several exams of this type, and the doctor was always alone. it is uncomfortable, but not painful. It doesn't last for more than a few seconds.
