When I give my teachers the evaluation they're supposed to fill out, I know I'm supposed to include a stamped envelope addressed to the college, but what do I put in the "addressed from" area on the envelope? Do I leave it blank OR do I put my own home address there? When I give my teacher the evaluation form, what should I include? A copy of my resume or a copy of the application, or both? PLEASE, help. My parents are completely clueless about these things!

Put your school's address as the return address.

(Remember to politely ask your teachers if they wouldn't mind writing a recommendation!) Give them a copy of your resume and be sure to include some things that give real clue to your personality, not just the clubs you were in or your grades, ex.: Your favorite book, or what you want to be when you grow up.

After they have written the recommendation, it's always good etiquette to give them a thank you note and perhaps maybe a tiny bag of homemade cookies or something- even if they are just pillsbury. (At least that's what I did). Remember: they are bombarded with these things every fall and by extending a more formal thank you it shows that you really appreciate the time they took out of their hectic schedules to write one of these for you.

you might consider sending them self-addressed envelopes with the colleges and your address on the envelopes, teacher evaluation forms, resume, and a letter about your major, interests, what points you want the teachers to make like leadership qualities, skills, etc... and put everything in one packet or folder so they won't lose it. then send them a thank you note and a gift for writing you a rec.
