Well, you've answered your own Q.
"I take a little time off." You have to be faithful to your routine and diet. Think of it as adopting a new way of life. Make your workout part of your daily habit.
Attitude has a lot to do with motivation. Again, your Q drops hints of self-loathing i.e., "my fat butt..." If you think in a positive way about your self-image, you will stay motivated.
It sounds like you have a slow metabolism, and I wd guess a stocky or heavy-set build. Work with this in a positive way. Build bulk in the upper body, and really watch your fat intake; don't par too much attention to the lean-looking guys, and you know what, they too are not totally OK with their images either, just watch their eyes as they go past the mirrors.
From time to time, yers, lapses will occur, b/c of family/work/travel obligations, and you can't make it to the gyms, so substitute an activity, like just doing some crunches.
If you think of fitness as a life-time commitment, and the adoption of a helathy lifestyle, not just for appearance sake, but your overall health, the small lapses from time to time will become inconsequential to the long-term plan.

Keep a picture of you looking good next to a picture of you looking "not so good" and put them on your mirror so you see them as soon as you wake in the morning.
This should give you a little more motivation.
Also think of going to the gym as putting gasoline in your car.....something necessary for function. Make exercise part of your day... like brushing your teeth...or taking a shower. Start out with just 10 mins at time walking. And work your way up to something realistic to YOUR schedule. If you say you're going to the gym 5 days a weeks for 2 hours at a time, that might not be realistic and you will quit. Start by going 3 days for 1 hr...for example and increase from there.
If you set the bar too high in the beginning you are setting yourself up to fail. Be fair to yourself.

I few years ago i injured my knee, and was not able to run anymore, it took me two years to find something that worked for me. What i found was that losing the first 40 pounds was easier to do with changes in diet, and once i lost it, i had a lot more energy, and it was easier for me to exercise.

Put a picture of a body you want...on your fridge and youll see it everyday. OR, put a picture of yourself on the fridge. OR both!
