If they kill themselves, blow themselves into bits, just to kill 20 people, would they care if they killed a million?
the answer is no.
And there are such things as suitcase nukes that aren't as strong as conventional nukes, but you don't need to put them on yourself, you can put them into a truck and run yourself into a building like they've done with regular explosives.

In the Middle east you must understand that the word "terrorist" or "terrorism" is used ONLY by Israel and the United States to define anyone who is defending themselved in one way or another against Israeli aggression, occupation, and genocide. There is NO other definition for the terms.
