Would you asipirate if you didn't suck out the fluid beforehand? How do you know?

Yes you would pull back on the syringe to check residual ( make sure the patient is digesting the feedings) the key is to put whatever you aspirate back into the stomach.. you can really mess someones electrolytes up by taking that fluid off.

A g-tube is implanted into the stomach so you can't aspirate anything. Aspirate is a word often associated with a function of the esophagus drawing a substance into the lungs instead of down into the stomach. People on a respirator ("trach tube") are another issue and, yes, those tubes are regularly cleaned.
