I've always believed that what you imagine in your mind is more frightful than the gore of recent releases.
The first Halloween.....(surprisingly little gore, but a lot of jump out of your seat moments).
The Exorcist.......(brilliant set up and great satisfying ending)
Seven......(I had nightmares about that stupid box for a week after seeing this film)

I grew up catholic so all of the Exorcist and Omen movies (originals) scared the crap out of me. To this day I can't watch the Exorcist all the way through and the music in the Omen still gives me chills.
As far as regular scary movies: The Shining with Jack Nicholson is the best ever!

The Final Destination movies and the Ring are all tied. The Grude comes in a close second because that little boy is CREEPY! I watched Scary Movie 4 and even in that, he's still creepy!
