Test of English as a Foreign Language.
In its most recent incarnation, it tests reading, writing, listening and speaking skills, and is administered by computer, over the internet, in authorized centers. The reading and listening sections are multiple-choice. The writing and speaking sections include both "independent" and "integrated" sections--the independent ones ask the test-taker to simply answer a question, while the integrated ones ask the test-taker to read and/or listen to a lecture and then summarize and respond to it.
Note that even the speaking part is by computer; you speak into a tape recorder (via a headset I believe).
Previous TOEFLs, which you may still be able to take in some areas, were the Paper-Based test and the Computer Based Test. Both included multiple-choice grammar sections, and did not test speaking.
EDIT: WTF? Ethan just copied and pasted my second paragraph!

Test Of English as a Foreign Language.
TOEFL evaluates the potential success of an individual to use and understand Standard American English at a college level. It is required for non-native applicants at many American and English-speaking colleges and universities. A TOEFL score is valid for 2 years, and then is deleted from the official database.

The TOEFL is administered worldwide by Educational Testing Service (ETS). The test was first administered 1964 and has since been taken by nearly 20 million students.
