Has anyone ever applied for a credit card and had it accepted... waited and waited but no credit card? Then you get a statement saying you have bought all these goods when you never even received your credit card?

I have never had it happen, but I am surprised. Most credit cards require you to call and activate the card using some personal information. For example, I get a new card every eight months or so because my wallet chews up the magnetic strip. When the card comes I have to call a number from my home phone. The company detects that it is my home phone and activates it. Is there anyone in the house who might have done it? In any case, if you did not make the charges you are not liable. Call the card company and tell them what happened. You won't be charged.

I did have that happen about 25 years ago. It's pretty much impossible today though. There are multiple protections against this type of fraud.

When you open a new credit card account, there are 2 followup mailings sent out several days after the card is mailed. The first one confirms that the card was approved and that it has been mailed and asks you to call immediately if you have not yet received the card. The second one provides your PIN for ATM withdrawls and again asks you to call if you have not yet received the card.

Finally as others have mentioned, you must activate the card by calling a toll-free number from your home phone and usually providing other information at the prompts. If you don't activate the card within a few days, they will follow up with more confirmation mails asking you to confirm receipt of the card.

It is obvious that the credit card you sent for has been stolen,and all the signs point to either someone working at your local sorting centre or who delivers your mail.
Firstly contact the Post Office and ask to speak with someone in the Fraud Investigation Department,pop into your local police station and report the theft and get a Crime Number,and then speak with the Fraud Department of the Credit Card Company who may ask you for a crime Number,they will then work with the Police and post office investigators.
You need to do this as a matter of urgency because,whoever has used your card probably still is.
