According to the novel, Frodo and the other Ring Bearers, except for Sam, sailed away to the Undying Lands in the farthest west across the sea. There they would live forever in bliss with the elves and the Valar, the Gods of Middle-Earth, who ruled under the authority of the Creator. There is a scene earlier in the movie, where Gandalf talks to one of the hobbits, Pippin I think about what happens to those who die (they are waiting for the start of the attack on Minas Tirith at the time). He quotes from what is actually the description of that final voyage into the west from the end of the story, part of which reads, "And then it seemed to him that. . .the grey rain-curtain turned to silver glass and was rolled back, and he beheld white shores and beyond them a far green country under a swift sunrise."
In the novel, Galadriel and her husband Celeborn also sailed into the west with Frodo, as well as Gandalf and Elrond, and, I suspect, Cirdan the Shipwright and many other elves, since the end of their time on Middle-Earth had come.
Galadriel, in particular, was returning home, since she had come to Middle-Earth from the Undying Lands of the Valar way back in the First Age to fight against Morgoth and had been exiled for thousands of years in punishment for disobedience to the Valar who had told the elves not to pursue Morgoth to Middle-Earth. In the millenia since then she had atoned and been pardoned. So the end is sad in some ways and joyful in others as all endings tend to be.

THough most of your answers sound good, he had to leave becasue he was a Ring bearer, in the books, Frodo,Bilbo, Galadriel, Gandalf,and Elrond left on the boat (not Celeborn as the movie showed). Also Sam was allowed to sail to the undying lands later in life since he for a short time held the Ring.

Frodo was physically, psychologically, and emotionally weary of the world. He carried a ring that had the power to overpower minds and hearts, stabbed by a cursed sword that nearly turned him into a Ringwraith, and was bitten by a demon in the shape of a spider. All of these contributed to deteriorating health. As a ringbearer(Like Bilbo, Gandalf, Galadriel and Elrond) he had earned the right to sail into the West. To tell you any more would spoil everything. Read the Lord of the Rings Trilogy.
