They are most often due to fetal abnormalities as others have said, but there could also be some health concerns for the mother. It's not your (or the mother's) fault, but it's worth getting checked out by a doctor to see if there isn't an underlying health issue. A family member of mine had multiple miscarriages and discovered it was related to an autoimmune problem. A friend of mine had multiple miscarriages and found out it was related to some ongoing gynecological problems. Both were able to eventually keep a pregnancy full term, but only by addressing the underlying issue.

Also, if the mother has had an abortion recently, there could be difficulties keeping the pregnancy due to stress and healing of the uterus. Some reports have suggested that having an abortion will have long-term affects on an ability to carry a healthy pregnancy full-term, but this is pretty disputed; most doctors agree that an abortion will make it difficult short-term, however.

Miscarriage, the loss of a pregnancy generally in the first 12 weeks, occurs in at least 15% of all pregnancies. It occurs much more frequently in the pregnancies of women aged 37 or older. Chromosome abnormalities cause about 50% of miscarriages but those with normal chromosomes have the same range of developmental problems.

The best answer to you question: If you have a serious consern, ask a doctor.

The next best answer; many many things. Blunt trama while pregnent. Infections. toxic chemicals. Some herbs - example; pennyroyal oil can cause hemoraging which leads to a spontainious abortion. There have been cases when people have used too much oil and the hemoraging has lead to death. Determining how much is too much isn't something to mess with.

A miscarriage is usually due to the baby not developing normal. The fetus may have had too many abnormalities and this was natures way of handling it.

Sometimes it is the body's way of getting rid of a fetus that has something wrong, or is not developing correctly. Sometimes a fall, or a disease, or physical inadequacies of the mother's uterus.

Most miscarraiges are natures way of stopping an abnormal development. Be it the baby, placenta or its placement. Most of the time, nothing you could have done or undone could have stopped it.
