Evaluation is usually something you do AFTER an action has been performed...
An Assessment is usually something you do BEFORE an action has been performed.....
i.e- You're an Emergency room doctor.. and a patient comes in and they are complaining of chest pains... You do your normal routine.. Assess the patient.. check all the symptoms.. then order some tests.. When the tests come back... you then prescribe your next course of action... A little later you come back to Evaluate the patient... Does that make sense to you?

Assessment
1. the act of assessing; appraisal; evaluation.
2. an official valuation of property for the purpose of levying a tax; an assigned value.
3. an amount assessed as payable.
In rough terms, both are very similar. Assessment has a more technical meaning. Evaluation is more medical, I guess.
In school, teachers assess a student to decide if they need special education assistance. But their work may be evaluated for a grade.

In education, evaluation is summative and usually used in describing a grade or measure given after a test, project, or course has been completed.
Assessment is usually formative and done throughout the class or course to give the teacher more ideas on how to reach the students better. The information is not used to evaluate or grade students, but to assist the teacher in teaching better.
