No, each deposit slip is supposed to have an identification number so the bank would know when and where each deposit was made and with what deposit slip too.

Yes. The deposit slip is only there to give certain info to your bank.
You can also legally print your own checks, even though the bank cashier might raise his eye brow, and a stupid little Junior immigrant might try to show you who is boss.
