Do you mean a division 1A or 1AA school? It has less to do with the size of the student body and more to do with the amount of money a school spends on its football program and the size of its football stadium (as well as attendence at games).

Depends on the state. Some states don't have enough schools to have more than 3 levels (e.g. West Virginia), whereas Michigan divides its schools into 8 levels.
