For really close friends I can goof around with, I say things like "hey loser" or "hi stupiiiiiiiiid". They say something similar back to me. It's all good natured.

For friends that aren't so close, just a standard "hey" or "yo" or "what's goin' on", something chatty like that.

With family, I actually usually greet them by name. "Hey (insert name here), what's up, so what're we doin'...." etc etc etc.

With people I don't know... well... I usually don't greet people I don't know unless I'm at work, and then I give the standard "answering the business phone" greeting.

I'm Italian, so I'm a hugger / kisser. I try to be more reserved with people I don't know well, but I feel awkward shaking hands. It's not natural for me! There aren't many people I don't like. I tend to avoid them, so I don't have to deal with any phony greeting.

For my family and friends, I say hi how are you and give them a hug or handshake. For people I don't like, I just say hi and get away from them as quickly as possible.
