All crochet pattern books will tell you that the needle size listed is approximate and to make a test swatch first to judge the accurate size. Some people crochet tight, some crochet loose, and this all bears a factor on the size of the finished garment. The size and type of yarn also bears a factor. There are some things I have made (such as granny squares) where I have had to use three or four different sized needles to get all the squares to come out the same size.

When reducing from a Barbie pattern to a Kelly size, you would probably go down three needle sizes. Bear in mind that if you also change to a smaller yarn (crochet thread instead of crochet yarn, for example), you may need to only go down one needle size.

It's a trial and error type of thing. Try a simple pattern first, and adjust needle sizes as needed. (The same number of stitches would be used, just smaller needles and/or smaller thread.)

I just went through the reverse. I had to enlarge my Barbie crochet patterns to fit a doll twice her size. Trial and error taught me to use regular 4-ply crochet yarn and two needle sizes larger. I got it done, though, and it looks great to me...!
