There are so many variations of this condition, you might just go to www.webmd.com and see your conditions and treatment. Surgery is a mean way to go, but if serious???

Its not exactly surgery, your MD can do it. He'll inject some local anesthetic into the toe and cut through the toenail to the cuticle and then cut away any infected tissues if there are any.

If your toe is not bad, but definitely ingrown, there are some ointments which can be applied to the side of the toe nail that help callus the side of the toe. Then if you remember to not cut into the affected side, you should be OK.

If you have persistent, infected tissue, or it bleeds, then it's time to do something about it.

Note: I would NOT recommend doing something about it yourself as Tigger has said, unless you've literally seen what a podiatrist or MD does to perform the procedure and you have the right instruments. I tried that once, but I couldn't cut the nail all the way to the cuticle. I think I needed better scissors. It sure relieved the pain temporarily, but the toenail came back even more ingrown (like by over a half inch!!). That's when I decided to go see the MD. I should have gone there in the first place, but my (then) primary care advisers (my parents) didn't think I should go to an MD for a chronic ingrown toenail, even though I actually had trouble with it regularly bleeding!!

I learned a lesson there. I was taught to learn the wisdom of your parents. But the fact is, your parents are not always wise. And its also not wise to do surgery on yourself.

This is not something I recommend, but I have done it because I can't afford extra expenses.

What I did was wash and dry the toe in question, then sterilize a straight pin and CAREFULLY pull the nail out. Then I trimmed it back as far as I could and doused the area with peroxide. The swelling and pain would go away fairly quickly after that.

Note, this is not something I would recommend. You would be better off seeing a podiatrist for treatment if it's that bad.

Trim properly straight across instead of down at the sides.

Try to cut it down as much as possible. If that doesn't work I would suggest going to a Podiatrist.
