6 tips for writing a personal apology letter:
Write this apology letter carefully on a piece of stationery; don't type it on a computer.
Express your apology in the beginning of your letter: “I am so sorry. . . .” or “I want to apologize. . . .”
State exactly what you did wrong early in the apology letter: “I shouldn't have lost my temper when. . . . ” or “I have looked all over, but I'm afraid I have lost your book. . . .”
Accept responsibility for what you did and don't blame the other person: “I accept full responsibility for what happened. . . .” or “I know this was completely my fault. . . .”
Promise in your apology letter not to repeat your offensive action and ask the injured party to give you an opportunity to prove this to them.

in an apology letter, less is more. is this a school assignment or did you do something to somebody? is this a professional apology, becase that's different, too. anyways, here's a good start:
Name of Person Here,
I screwed up. (depending on relationship - professional/personal, etc you can change that to messed up, f'd up, made a terrible mistake, etc.) You don't have to forgive me. You don't. I don't expect you to because what I did was terrible.
I'm sorry.

Well, i wont write it for you, but here is something to start on...
1. Say what you did
2. Say what you will do to fix it
3. Say why you won't do it again
that should be a good template. also i dont know what you did so i couldn't write it even if i wanted to. but if you follow whats above you should have no problem writing it.

Do your own homework.
Try to remember when you had to say you're sorry for something you did. Then just write out what the situation was, describe it in writing and you'll have it done!
