My 17-year old brother wants to change his name since he was named after his father, who he hasn't seen or been in contact with over 12 years.

Without the permission of BOTH legal parents, he cannot change his name while a minor. Once he is 18, he can legally change his name without parental permission. A new birth certificate will be issued with his new name on it, but it will still show the names of both legal parents.
