Isolated Thunderstorms/Weather are in simple terms dense groups of clouds that usually are found to be very dark in nature but sometimes just grey and can precipitate large downpours of rain to an "isolated" region.
For example, in a metropolitan area, an isolated cloud system can begin to precipitate in one side of the city but not on the other. In some cases, the other side of the city not experiencing rain can be in sun and cloud and not be aware of what's happening on the rainy side. Isolated thunderstorms/showers usually develop during warm periods where an instability occurs causing a quick mass of clouds to combine and rain in a particular area. You can sometimes see this occur on a partly cloudy and sunny windy day where a dark cloud would simply hover over the area and either begin to hail or rain heavily. During and after the precipitation usually follows lighting & thunder.
Scattered Thunderstorms/Weather occur in a much larger region and can be expected to cover an entire city with a few breaks in between. Scattered thunderstorms usually roll over a city in a much larger area than a isolated storm and brings precipitation and lightning in a more even distribution.

Probability...kind of! We have our precipitation forecast served to us in two ways, in the form of percentages and descriptive phrases. Chance of showers, scattered showers, occasional thunderstorms, isolated thunderstorms... It can be confusing. So what does it all mean? Well, the National Weather Service calls it POP (or Probability of Precipitation). We are talking about "showers" in this case, they break it down like so:
1. Slight chance, random, widely scattered, isolated = 10-20%.
2. Chance or scattered = 30-50% chance.
3. Likely or Numerous = 60-70% chance.
4. Occasional, periods, or "it's gonna rain" = 80-near 100% chance.
Interesting that today's forecast on weather.com says a 30% chance of thunderstorms but describes the chance as "isolated".
