A night to remember.
Dinner at her favorite restaurant, or some place romantic that you've picked out. Call ahead and have a dozen roses waiting at the table. You could rent a cool car for the evening. Bring her a small gift when you pick her up, but then have the waiter deliver the NICE ONE when he brings dessert. If it's not too cold, see if you can set up a carriage ride. Write her a birthday poem; don't worry how it sounds, she'll be impressed that you took the time to write something from your heart.
Most of all, do something that will make her feel special and unique. I once had a coffee cup made for a friend from a custom design source online. I made an avatar (from Yahoo!) to look like her, added her company logo, and printed "hello" in her cultural language.

An engagement ring in the shape and size of a porsche or mercedes or BMW or Lamborghini. lol 21 year old women usually love shoes and clothes and gift vouchers from a very nice boutique are perfect.Oh yeah, you could start by calling her a woman, not a girl.

It depends on how much you have got to spend!
But a nice cheap idea is a champagne glass with her name on it!
I did it for my frinds 30th and it looked lovely!
You can also get a bottle of champagne with her name on it!
A nice bootle to keep once you've drank the contents of course!
