Off the top of my head I know that both The Crown and The Omen (original version) were riddled with accidents and fatalities. There're probably other ones as well.

Twilight Zone directed by Jon Landis. Actor Vic Morrow died, he's Jennifer Jason Leigh's father & two asian actors i'm not quite sure of their names. One was a young boy. They died when a stunt involving a helicopter went wrong & crashed on the set killing all three instantly. The movie still premiered but without the helicopter scene.Jon Landis was sued for reckless endangerment & I believe the charges were eventually dropped.

"the ten commandments" - extras drowning and that sort of thing.

also the surfer movie "Point Break" - the stuntman died during the filming of the "catching the big wave" sequence during the storm at the end. They still used that footage in the film.

I know that the film was cursed from start to finish with plane crashes and strange goings on!
