Nothing, It's not like those people know me. Well most of them don't anyways. Plus they wouldn't be working there if they minded it one bit. I used to work as a cashier before and I never cared what anyone bought and neither did any of the other cashiers I worked with.
I did find out that when I was I was in school ( not to long ago) when the teachers would come in and buy beer they would get embarrassed when their students were working the registers.

I never really get embarrassed, so one of my good friends begged me to pick up some Monistat for her. I said "No problem," because I just didn't think it would be a big deal.
But once in the store, it did completely embarrass me! I picked up a whole bunch of extra stuff I didnt really need, hoping the woman at the counter would mindlessly scan and bag without really looking.

I'm past the age of getting embarrassed over much of anything but......I did feel a little uncomfortable recently when I had to run into the local market to buy adult diapers for an elderly relative. I was sure that someone I knew would think they were for me.

Condoms.. my boyfriend gets red in the face and all embarresed everytime we have to buy them. He'll start laughing and fidgeting .. It's ridiculous.. we buy them online now just to save us the embarresement.
