Short answer: It rearranges files for quicker reading and writing.

Long answer: The hard disk is divided into units called sectors. When you go to write a file to the disk, the hard disk throws it into into the first available sectors. Consequently, the disk ends up looking like Swiss cheese. Files are spread piece by piece all over the disk. The hard disk keeps track of these with a FAT or File Allocation table. When you run the disk defragment (see below for instructions) it will analyze the chunks and move them into more continuous pieces. The end result is that the disk can read the entire file in a few less rotations than before. Defragging also fixes cross-linked sectors, and errors in the FAT table. Modern disks run between 5400rpm and 7200rpm so this is not really a speed improvement, it is more a stability improvement.

To launch disk defragmenter (admin rights required):
Click Start > Right click on My Computer > Manage
Click disk management > analyze

this will determine weather a defragment is appropriate.

It is kinda like this, but only kinda... the disc is spinning and as you save files the computer drops the files where the spinning disc is at leaving space between drop files. So when accessing the files the computer has to go by the empty spaces also. The defragmentator movers the files together removing the empty spaces. Do you know how to get to the defragmentator?

All of the answers so far are good and correct. It's basically the elelctronic equivalent of reorganizing your filing cabinet to be more efficient.

Something I've not seen anyone mention is running Disc Cleanup first. It's good to get rid of all the temp. internet files and history from time to time.

And, if you're new at the process, it might be good to create a Restore Point before you clean up or defrag.

I clean up about once a week and defrag about once a month.

Wish I could say my home were as clean as my puter, lol.

The disc defragmenter relocates the files stored in your computer so that they are easier to access. It usually allows your computer to run faster. It's a good thing, theres no harm in using it.

It cleans up your files on the computer, gets rid of empty folders, and sorts the folders in proper order.

It does this to keep your computer running smoothly.

You should defragment about once a month.
