You should use the leaf on the entire breast. It is reccomended to leave them on for 20 minutes and repeat 2-3 times a day.

1. Use green cabbage.
2. Wrap the cabbage leaves around the breast and leave on for about 20 minutes. Twice daily is enough. It is usual to use the cabbage leaf treatment two or three times or less.
3. Stop using as soon as engorgement is beginning to diminish and you are becoming more comfortable.
4. You can use acetaminophen (Tylenol(tm), others) with or without codeine, ibuprofen, or other medication for pain relief. As with almost all medications, there is no reason to stop breastfeeding when taking analgesics.
5. Ice packs also can be helpful or a hot shower were the water hits your breasts.

I'd put it all over, hey, the more the merrier. They also make special pads with "cabbage" in them as well as some with the "lavender" and whatever else they say to try. But to be totally honest. If you're doing this to help prevent/cure engorgement, the only, and i mean only thing that worked for me was wearing the tightest sports bra I could manage to put on, plus wrapping my breasts with ace bandage as tight as I could. Then everytime I had a few minutes to sit or lay, ICE ICE ICE. But (if this is what you are trying to do) hopefully you'll be able to find some other ways to soothe.

I am assuming that you are engorged. yes you can use them all day over the nipple area. Also try intoducing some peppermint into your diet it causes the reduction of yout milk supply. Hang in there it takes about 3 days to change your milk supply either way. If it tis really uncomfortable try using frozen peas as an ice pack on the breast they comform to the shape well. just mark them with a sharpie so you don't eat them after thawing and refreezing them.

over the whole breast . it works to minimize swelling and discomfort when trying to stop breastfeeding . change them as they become hot -the point is to keep your breast cool and minimize inflammation .It took me 3 weeks for milk to dry up.
