That's disgusting. Worse than disgusting; I can't find a word. He should feel guilty. He should feel guilty for the rest of his life. There is no absolution for this; no way out. It's the problem with the south; everyone here practices multipurpose relatives. Their sister is their wife; father is their brother, mother is their aunt and sister and grandma. They are all butt ugly and stupid to the point of bring retards. Animals. That's what this kind of thing leads to and it should be a legally punishable offense. Unacceptable. Feels guilty? He IS guilty. Oy g'valt!!!

Think about the root causes of the incest taboo: 1) preventing birth defects in children, 2) keeping separate family-of-origin and family-of-choice roles. The feeling of guilt is useful insofar as it prevents him from, presumably now as an adult, reenacting sexual acts that could lead to children having birth defects or confusing family-of-origin and family-of-choice roles. After that, the feeling of guilt isn't useful and is something to try to let go of.

When you say "all acts happened due to age," I assume that this is something that happened when they were both quite young amd near the same age--not that he was significantly older than her, in which case there might be more emotional damage on her part. In which case, my next recommendation, therapy and trying to talk about it with his sister, would be even more important.

Therapy is great for trying to work through stuff like that. And this might be something to venture into after being in therapy for awhile, but maybe also talking about the feelings of guilt and anxiety with his sister, who might also be having those feelings.

If he was significantly older her, making it more of an overpowering thing and less of a mutual thing, talking to his sister and expressing remorse would be really important in terms of helping her heal.

They both need serious counseling. By a professional and/or religious figure.

I suggest psychological counseling. That's pretty serious for both of them, and also unacceptable behaviour.
