I have tried the office supply stores w/ no luck...I just want the correct form .the local courthouse here in Greenwood SC donot have the forms...basically there is a spelling error on his first name my son is 15 years old and to make a long story short he wants it corrected.From Loren to Lorenzo...w/ the last name staying the same...I just donot want to have to pay an attorney fee to do the paper work...I would rather do it myself and pay the filing fees

I don't know why South Carolina isn't as up-to-date and computer literate as California, which has the form on-line as a fillable PDF: http://www.courtinfo.ca.gov/selfhelp/other/namechangeform.htm#namechildpar

Here's the form you need, for free, from a commercial Web site: http://www.ilrg.com/forms/namechange-minor/us/sc

(You could buy the form here http://www.namechangelaw.com/states/sc/southcarolina.htm for $35, and maybe save some typing and/or formatting, but I think that's a gyp price)

How odd that your local courthouse does not have these forms. Try the DMV next, as they often handle name change issues. They can at least give you some free advice, at least this is how it works in CA. Next to this I do not know what else to tell you. You may have to contact an attorney's office and ask if you can get the forms from him/her. Contact a female's office and see if her office will just give them for free or a small fee. Should work, but I am not sure of SC law. But it is worth trying at least, right? Nothing tried, nothing gained and the worst you can get is a no, and if you don't ask it is a no anyway.

Try calling your local court office. They are listed in the phone book under Government. If there is no listing for Family Court, where you need to go, try the Superior Court. They should be able to lead you in the right direction.

If you do not do this by the time your son is 18, he will be able to legally change his name himself at that point. Taking care of it now, before he starts preparing for college, is best though.

Ask your County Clerk. I don't think there is a form, but they can tell you what you need. It probably should be the same county where the child was born.
