I lost account many reformats ago...

That's a regular "mantainance" procedure for me, because Windows accumulates lots of garbage on everyday use, and applications and upgrades both in Windows and Linux (the OSs I use) reach a point where more space is needed on every system, apps or docs partition. So, as a minimum, I reformat once a year, more precisely, I repartition harddiscs and reinstall.

My endline is when time comes tu upgrade to next OS version (to get rid of cummulative human-bound errors or distributor's bugs), or to next work software suite.

Nº of times: I can't remember the number of times I've formated.
Reasons: Generally I clean before I format. I format when it's not possible to recover the system, and only after I've tried my best to recover the info. I also format when I want to install another system and I have no other way of getting space. Work is another one.

OS removed: Win98, Win ME, Win 2000, Win XP Home, Win XP Pro, Mandrake linux, Ubuntu linux, FreeBSD, Suse linux, and I might've removed a few more.

OS installed: Just about all of the above, except Windows XP home. The linux are all multiple versions in multiple years. FreeBSD is also multiple versions in multiple years.

No. of times : Probably about 10 times
Reasons: Mainly updrages, but with older versions, system errors
OS removed: Win 98, Win 2k, Win XP
OS installed :Win XP

Win XP is extremely stable. I have not had to re-install it or had any errors in the 2 years I've been running it. But, I know how to get rid of viruses very well and run two firewalls.
