I don't know whether it sounds correct , but
Yours faithfully, is correct with a comma after faithuflly and this should be signed only in the letters we address formally with a Sir, without name in the address form.

Sir,
---
Yours faithfully,

xxxx

Yours faithfully is correct (as per formal letter format).
However in an informal letter it can be-
1) Yours affectionately
2)Yours truly.

Yours faithfully and Faithfully yours

"Yours faithfully" is correct. "You'rs" is a contraction of "your" and "is." Literally that would be "Your is faithfully." That doesn't make sense.
