Although the length of time could vary considerably, here's a typical timetable, assuming the student already holds a Master's degree.

2 years of coursework
1 year preparing for, sitting for, and passing language exams and comprehensive exams (sometimes known as qualifying exams)
2 years researching, writing, revising, and defending the dissertation

This timetable could be extended by a bit (a year or more) if the student is teaching full-time during dissertation research, or if dissertation research includes lots of archival research, or research abroad.

If you're good (great GRE scores, superb letters of recommendation from your professors, stellar GPA, and strong preparation in your area of study in terms of undergraduate and Master's coursework), it shouldn't cost you much. Top candidates are offered fellowships and/or teaching or research assistantships which carry full tuition remission and a small living stipend.

Most doctoral students follow the money," meaning that they choose the program which offers them the best support.
Sometimes, however, students choose the program with the strongest reputation and best professors in their specific area of study, even though they are not fully supported. This can cost a LOT, and result in over a hundred thousand dollars in student loans.

(You asked "what courses" a PhD student takes. That's impossible to answer, since it completely varies from field to field, and from student to student. It depends on what the student's specific discipline is, which university the student is attending, and which courses the student chooses.)

It all depends on the field, university, and you. Some Ph.D's take 4 years and some up to 10.
