mix a solution of mild bleach and spray it it will kill the mildew and the smell with it. lysol does ok for the airborn particles but use a fan to bring in fresh air while you're doing this.

It's really hard (if not impossible) to get mildew & it's odor out of fabric.

When we were young, our mother would sprinkle clothes she planned to iron and put them all into a plastic bag. One time, she forgot. The clothes developed mildew. No matter how many times they were washed, hung on the line, starched and ironed afterwards, I remember that dress always had stinky sleeves.

I would try that enzyme for pet odor Kim recommends. It might be stronger. Check to see if it is OK for wool yarn. Handle the rug gently because wool tends to becomes more fragile when wet. That is why Woolite soap was invented.

First vacuum any loose debris-
then mix one part vinegar to 3 parts water and put in spray bottle.
Spray onto rug and let dry.
If this does not completely get the odor out repeat.
Wait till it dries, then cover carpet in baking soda to help absorb any residual odors.

You can also try an enzyme cleaner such as for cleaning pet mess (I think "OUT! stain and odor" is the best) You can spray/pour it on, let dry and everything is perfect again
