Oh, let's see, Language, Violence, Strong Adult Subject Matter... Of course, it was also released in 1975. I don't know if it would still be rated R today, but it wouldn't surprise me. Likely though, it would probably make it to PG-13 by today's standards.
...Add-on in response to michael_c: I disagree, I grew up in the 60's and 70's, raised kids in the 80's and 90's, and I think there is a much broader level of acceptance today. And when this movie was released, there was no such thing as PG-13, no middle ground between PG and R, which likely pushed a fair number of movies into the more restrictive R rating that would garner a PG-13 by today's standards.

I don't know - I have not seen it - but a message to 'dangermom' that they rate movies alot stricter now than they did in the 70's. I grew up in the 70's and 80's and saw more nudity and heard more foul language in the PG movies back then, than I see and hear now in the R rated movies.

During the era of the film, open conversation regarding the psychologically ill was deemed inappropriate.

I guess the content of this movie may be too "heavy", and/or confusing for younger people.
