1) start with organizing your desk, your room. the space that you use to study. clearing up physical clutter can give you a more peaceful place to function in. it also shows your determination to get yourself all organized. everytime you walk into your room, you visualise the "organized you". it will be a constant reminder.
2) organize your schedule. if you are not getting enough sleep, you would not have enough energy to face you day. not to mention the number of responsibility you have. instead of going to bed late, go to bed early and get up early. start you day early instead of ending your day early. your brain can function better when you are well rested. you can be more efficient and productive. if you are studying at bedtime, not only you deprive yourself from sleep, but also studying at a time that your brain simply can't absorb that much.
3) schedule free time for yourself to do things that you like to do. don't skip this important part. make time for yourself while you have no time. that way, you can finish things with a deadline so you deal with your responsibility more effectively. then you can go do whatever you want. it will make you happy and help with your stress level.
hope it works for you

Wow, you have alot on your plate. Well, I'd say get rid of the things that are least important. Looks like you have alot of "must-do's" in your schedule; I don't know how important it is to you, but maybe hold off on the working out for a while. Maybe go a few weeks with only a schedule of school/homework-work-sleep. I don't know where you work, but you may be able to get in some homework or studying while on the job. Try seeing if you can integrate some of your "to-do" things.
