It means TANSTAAFL, or "there ain't no such thing as a free lunch". In other words, everything that is good or extra in life has a price or a cost. You are entitled as a human being to have shelter, food, clothing, and to be safe, and you are entitled in America to a free public education. But other than that, anything else is a luxury.
"... [E]verything is a luxury, starting with being in the world" seems to imply that it is lucky to be born, so you need to make the most of your life. Be ambitious and try to live the best possible life you can live. If you want material things or money, realize that they come with responsibilities to care for and maintain them.
So this seems to be saying, don't expect the world to hand you a good life. Don't expect good things to just fall into your lap. If that happens, great, you're lucky and have been blessed, but now you have a responsibility to be grateful and to use your gifts, talents and knowledge to help yourself and others.
I believe we are put on the earth for a reason, that we have specific tasks here that we need to accomplish. We may have issues and concepts to figure out so that we can grow. We are not human beings on a spiritual path, but spiritual beings on a human path. This saying isn't mine, but it points out a truth: be grateful you're here, and leave the world a better place than you found it.

It means that everything has a cost whether or not it says it is free your paying for your life everyday for example even though people say going to school like high school is a free education what you dont know is that your paying for it with your taxes every year so although you dont pay upfront ur still paying for that luxury and being alive and living on earth is a luxury you pay for by working or doing whatever you do everyday.

i think this means that you pay for everything that you do. including living. i mean, it says everything is a luxury and that you may for all luxuries, so i think that its saying every single thing in this world is paid for in some way or form, whether it be physical or emotional.
