The largest chain Pret A Manger, the sandwiches are put together by hand. However, the fillings are mass produced and distributed to each store.

I believe most are hand made, but then I knew one or two guys who were employed by these companies, and trust me you would not eat anything that may have come within a mile of them! eeeewwww!!!
