Different ways. The most common is when the ref makes an X over his head signifiying that a wrestler is really hurt but the WWE is starting to use this in the storylines now.. Another is by listening to the annoucers. If they sound confused and don't know what to say or aren't saying anything at all its because they don't know what to do because it wasn't planned. (At least Cole finally shuts up this way.) Same thing with the wrestlers. If they look confused or start to take a long time to taunt and the ref checks on the other wrestler, that is a way.

If a wrestler gets injured from a move that went wrong, the rev will cross his arms above his head to signal the trainers in the back that it is a real injury.

I don't know. It's kinda hard ta tell when it happens right then. When u hear of a superstar that got injured, that's when u know a stunt went wrong.

Did you see the brock lesnar vs kurt angle match at wrestlemania a few years ago? If you did then you'll se that lesnar messed up a 450 splah. He landed on his head and he Just got really messed up. He was unconscious and things just went really wrong.

Usually when someone lands really really badly on there head or Neck Area. Usually it will be slow to get up and sometimes they call the match or quickly cover to get it over.
(if it looks flat out wrong. It is)

The "superstar" that messes up usually gets "suspended" or "fired" on world WWE television. And in the worst case scenario he/she dies... :(
