Take a trip to your local post office... Have about $150 handy and a few months to wait (or expedite for additional charges)... And don't travel to The Middle East nor Asia (for your own well-being)...

Get an application and instructions at the Post Office. You will need a certified copy of your birth certificate. Also, most couty clerk have an office to.

I just got one. Go to your local post office and they will sit you down and help you out. You need a two 2" pictures of yourself that you can get taken either there or at a Rite Aid, Walgreens, CVS, etc.

In Japan? In the USA...go to a post office for instructions.
My friend is from Japan and she said there is a "Pass Port" office in your local City Hall if you're in Japan.
