No, the cast on row does not count as the first row. Row 1 is the first row where you actually knit. As for which is the right side in garter stitch, yes they look the same, but it you're keeping track of which is the right side (say for instance your pattern says knit until 10" long ending on the right side) it'll be all the odd numbered rows--though this usually isn't an issue.

In garther stitch, both sides are the same. Depending on how you cast on, but almost always, the cast on row countsas a row.
